Incident Reporting
How to report an incident
Please use the form linked below to report any kind of incident within the district, including but not limited to incidents that you might consider as: bullying; civil rights violations (Title VI incidents); sexual, sex-based, or gender-based harassment or discrimination (aka Title IX incidents); or, any other incidents warranting an investigation and resolution by the school district.
Upon receiving your report, school and district staff will: a) promptly review the initial report and reach out to you with next steps (if you provide your name and contact information); b) reach out to the target(s) to take steps to ensure their safety; and c) initiate an investigation or inform you that an investigation was already completed relevant to the incident reported.
If you do not wish to complete the form below, you may instead email a report to incidents@mgrsd.org or leave a voicemail at 413-458-9582 x4000. Paper copies of this form are also available in each school's front office.
Please provide as much detail as possible for the district staff to use in responding to your report.
